My opinion the ladies can be takes all represents, including doctor and surgeon, so I think ' T' is the right answer.

It is asked in the question which letter represents ladies who are both doctor and surgeon. So it is clear T falls in all the 3 figures if we observe carefully so answer is 'T'.
