From the given question in ladies only they are asking who doctor and surgeon both are represented so the answer is T.

It is asked in the question which letter represents ladies who are both doctor and surgeon. So it is clear T falls in all the 3 figures if we observe carefully so answer is 'T'.

From the question only point out the ladies and surgeon only not the male doctor so ladies doctor and surgeon in present in the T option B is the correct answer.
