Option D will be the right answer has both are effects of something that happened. B cannot be the right answer as hospitals don't prevent diseases they cure it.

I think both are independent effects because there is a decrease in no of people affected that means there is precaution taken by the authorities to decrease the affected people that means the decrease in no of effected people must be an effect.
