Yes I have remained absence for past 15 days as I have been a severe alcoholic patient in past whereas it is disease of emotional and mental disorder I had been rehabilitated few years ago but during the undertaken renovation work of my residence I possessed some financial difficulties as well as I had to manage my family my job and the responsibility of repairs and maintenance work of my house in the scarcity of time by means of which I had been affected with mental imbalance which raised my alcoholic consumption and total lapse on duty. Inconvenience is regretted and as this is stated fact you are requested to take a stand of lifting hand toward the honesty and extend your support toward to overcome a disease of a suffering man.

Yes Sir/Mam, Sometimes when I have been ill and if there is some urgency. But even though I have seen to it that the work is not affected to a major level. If I have been ill and not present at the office then I make sure that I am available in phone. And If I am taken leave for any other reason prior I have always explained the work to the concerned to be done in my absence.

Yes I have taken leave for 2-3 days but I have completed my work properly so that there should be no pending work. And told to my Team Leader about the leave before 2 days because that was an urgent leave because my mother is not well and she was in Hospital and no one was there to take care of her. So I took urgent leave for that.

Nope, I never been absent from work more than a few days, and as I said I always give first priority to my work and I'm purely dedicated to it from past two years.

And I always make sure that my leaves are planned and I inform about it to management at least 15-20 days before so that a back up can be arranged for that time.

Yes. I have taken more than few days leave. But, I have informed about my leave before 2 weeks to my superior officer and to all my team-mates those working on the same project.

Also, before leaving I had documented the work completed by me in that project to help others team-mates to continue my task during my absence.

Yes sir, as there are circumstances in which you can't afford to not spend a few days to take care of (like a marriage), but the important thing is you have clear communication with your colleagues, your customers and prepare a good backup plan to make sure that the company business is not interrupted in any way.

No sir, I never took a long holiday break in my previous projects. But, being a human, we may face some emergency or family-related problems, At that time I will consider this, besides I will never plan long holidays that could affect my assign work.

Yes, depending upon the situation means I took leave at an emergency only, post seeking permission from my boss, simultaneously I prepared a backup just to ensure smooth running if business and no escalation and complaints during my absence.
