This is caused when the gravel, cement and water are not properly mixed together. When the water content is too little, and the concrete is poured into the structure, the mixture will not settle properly causing the water to drain from the top and settle on the bottom. This would result in the small holes appearing on the surface.

Segregation is due badly mixing of concrete. Bleeding is over compaction of concrete via vibrator etc and honey combing is a result of Lean mixing of concrete of improper compaction while pouring the to congestion inside the concrete body.
