Segregation occurs due to pouring from height and no compaction.

Bleeding happen due to excess water (increased W/C Ratio).

Honeycombing occurs due to improper compaction & W/C ratio.

As per my knowledge;

When badly mix concrete is used in structural works then its gains less strength due to honey combing which causes the deterioration of the structure.

Segregation should be the answer, if the mix and the proportion is bad, we will have segregation.

We mixed properly, but we are not compact properly, we will get Honey Combing.

Segregation has happened during the pouring or placing of concrete from more than 1m height.

Bleeding is happened due to excessive water.

That's why honeycomb is the right answer.

Segregation also occurs due to lack of binder material.

Honey combing is the right answer because segregation is due to improper placing and bleeding is due ti an excess amount of water when we have to mix in the concrete mixture. So honeycombing is the right answer here.

When concrete is poured into the foundation forms it does not just flow in like water and fill up the forms to the top. If it is not vibrated properly it may leave voids called honeycombing.

I think honey combing is due to improper compaction of concrete.
