Honeycombing is due to bad compaction. Not from the bad mixing of concrete. I think segregation is the right answer.

I think option D is right because honeycombing Is occurs due to less compaction, bleeding occurs due to excess of water and segregation is occurs due to all mixture should not mix properly but this question is wrong because of badly mixed means many types.

Segregation main cause when less binding material use and poorly graded coarse aggregates are use and water use in more quantity results separation of coarse particles.
Bleeding due to excess water and due to improper mixing. Concrete leads to honeycomb as. Pores are left. In concrete improper vibration. All leads to honeycombing.
