In my opinion, badly mixed cement concrete result is honeycomb, segregation, and bleeding.

Honeycombing is also happen when it is not properly compacted. But I think all d options are right.

Honey combing is caused by insufficient fine material in the mix, perhaps due to incorrect aggregate grading or poor mixing. This can be corrected by increasing the sand and cement content of the mix and by proper mixing, placing and compaction.

In concrete, there are two types of segregation.
1. Course aggregates are separated from the concrete.
2. Groute (cement+paste) is separated from the concrete.
Honeycombing is the right answer.

Segregation occurs when less water cement ratios due to dry of concrete, bleeding occurs when high or excess W/C but honeycombing occurs only a bad mixing process happened so honeycombing is the correct answer.

I think segregation is the right one because it occurs due to wrong mixing and in much water ratio also.

When concrete is poured into the foundation forms it does not just flow in like water and fill up the forms to the top. If it is not vibrated properly it may leave voids called honeycombing.
