I agree, There is no clear mention about the gender of Suresh. In question, it is only mentioned that the boy in the picture is the only son of Suresh's mother. But by only son, it does not mean the only child. Hence, Suresh may be a girl also. Hence we can't assume it.
Therefore in my opinion the question is incomplete or there should be more options in the answer section.

The answer is father as the only son of my mother that the only son is he himself and his son will be the only son of the son of my mother. So Suresh is the father of that boy.
