In this type of questions, first, you need to find the word anywhere in the sentence ( is/as/was) and then at the palace of is/as/ was you put one equal symbol(=).
Then fix the equal symbol as reference , in right hand side apply the backward approach and un left hand side apply the forward approach.
Ex: He = the son of the only son of my mother.
Now go to the word my ( here talker is Suresh) I,e. Suresh says my mothers only son.it means Suresh is the son of a mother.

Which statement tells you that suresh is a male? Suresh can be a female. Merely having name 'suresh' doesn't specify whether it is a male or female. So if suresh is a male he would be father of the boy in photo and if suresh is female she would be paternal aunt of the boy.
But after looking at the available options only correct option is father. So the given answer is correct but given explanation is wrong.

"He is the son of the only son of my mother." Please observe the sentence that mentions "only son of my mother" it means mother has only one son that is suresh, hence the son (in photograph) of suresh means-Suresh is father of that boy.
If "only" would not have been there, it will mean mother would have more than one child (suresh) then answer would be different.

I agree, There is no clear mention about the gender of Suresh. In question, it is only mentioned that the boy in the picture is the only son of Suresh's mother. But by only son, it does not mean the only child. Hence, Suresh may be a girl also. Hence we can't assume it.
Therefore in my opinion the question is incomplete or there should be more options in the answer section.
I agree, There is no clear mention about the gender of Suresh. In question, it is only mentioned that the boy in the picture is the only son of Suresh's mother. But by only son, it does not mean the only child. Hence, Suresh may be a girl also. Hence we can't assume it.

As per the question, Suresh is the speaker. He says the only son of my mother. He clearly mentioned that it's his mother and the word only son means one son. So there are no chances of two or more sons. Suresh is the only son of his mother. The boy is the son of the only son of Suresh's mother.
Son of Suresh's mother is Suresh. Son of Suresh is the boy. Now, Suresh is the father of the boy.
Son of Suresh's mother is Suresh. Son of Suresh is the boy. Now, Suresh is the father of the boy.

In above question, the boy is the only son of the son of my mother it means as per second generation the Suresh is the son of his mother which has been clearly mentioned in the question and that boy is the son of Suresh.
Therefore Suresh is the father of that boy and his mother is the grandmother of that boy.
Therefore Suresh is the father of that boy and his mother is the grandmother of that boy.
