                He is not coming tomorrow since / because(in place of 'as') he is having a pain in the chest and has to see ... It is obvious that he will see the doctor, not me, eh? Therefor the third 'he' in a single sentence is unnecessary.

                @vertika: In your sentence, the is used before "severe" and not before pain. So " a pain ", I believe is not correct.

                "a pain " is a wrong usage. And one should 'consult' a doctor, but not to see a doctor.
Example: Chest PAIN, you should CONSULT a doctor.

                Here the right answer should be b as.
Sentence given to us - as he is having 'a' pain in his chest.
Hence the correct answer won't have an 'a' as it can't be used before pain.
