"Pain" can be both "countable" and "uncountable". If you are referring to a "specific pain" then the "countable form" is more likely.

"a pain" is wrong. 'a' cannot be used before uncountable.
"see doctor" is also wrong. It should be "consult doctor".

"a pain: is wrong usage. It should be "pain in the chest". I dont think, we can use an article(a, an, the) with pain.

I feel both b and c are wrong.
b- should be " as he is having a chest pain"
c- should be " has to consult a doctor"
