"a pain: is wrong usage. It should be "pain in the chest". I dont think, we can use an article(a, an, the) with pain.

I feel both b and c are wrong.
b- should be " as he is having a chest pain"
c- should be " has to consult a doctor"

Option C can only be that is given as its answer.
Rest all is correct. "a pain" is correct.

Pain is an uncountable noun so it can't be used with "a".

No because he is having pain could be correct but as he is having a pain is not wrong.
