Option "D" seems to be most opt to me, since there's no need to use 'he' multiple time and the sentence is correct without it!

Because is used in place of as. As/for used for weak case and because is used for strong case. Here he has a strong reason.

He is not coming tomorrow- 'is' used because of (not coming is conform action in future for that we use simple present).

"a pain" is wrong. 'a' cannot be used before uncountable.
"see doctor" is also wrong. It should be "consult doctor".

@vertika: In your sentence, the is used before "severe" and not before pain. So " a pain ", I believe is not correct.
