I think it shld be: error in A and C both:

Here we are talking about thing that happened in the recent past, so it will be fell and not fall.

Had + verb 3rd form in the if clause is always followed by would have + verb 3rd form.

I think would is appropriate because imaginary or may happen things must use would.
