I think it shld be: error in A and C both:

He fall from a running train and would have died if the villagers had not got him admitted in the nearby hospital

Here we are talking about thing that happened in the recent past, so it will be fell and not fall.

Anybody can tell me why in part(c) "did not" wrong and why "had not " is right.

Had + verb 3rd form in the if clause is always followed by would have + verb 3rd form.

What about B? It would be "could have been died".

I think would is appropriate because imaginary or may happen things must use would.
