There are three situation in one sentence.

1) if you don't keep quite " there is an order "
2) i shall shoot you " there is a clear action "
3) He said to her in a calm way " there is an action ".

So answer would be in a past sentence as well as in the combination of three situation. That's way answer "C" is correct.

In direct-indirect speech, present tense must be changed to past, here option A & option C is in past, option B & C is removed and in option A, CALM is not mentioned. So OPTION C is Accurate.

Calmly is used because the questions say that "he said to her in a calm voice". So, we have to include the tone of the statement to indicate the manner of saying.

Yes, option C is correct because the tense is to be changed into the past but not to the future.

Answer 4 is wrong because it is in future tense, present tense is change into past tense in change of speech.

Reported verb -that is used in imperative sentence will changes to would and shall changes to should.
