Calmly is used because the questions say that "he said to her in a calm voice". So, we have to include the tone of the statement to indicate the manner of saying.

There are three situation in one sentence.
1) if you don't keep quite " there is an order "
2) i shall shoot you " there is a clear action "
3) He said to her in a calm way " there is an action ".
So answer would be in a past sentence as well as in the combination of three situation. That's way answer "C" is correct.

Reported verb -that is used in imperative sentence will changes to would and shall changes to should.
