It has 3 terminals like JFET. The substrate is the extra one.

If Vsb(source to body voltage) = 0. Then 3 terminal.
If Vsb(source to body voltage) ≠ 0. Then 4 terminal.
If Vsb(source to body voltage) ≠ 0. Then 4 terminal.

Substrate is not terminal it is material (sio2) etc. So answer is only 3 terminal.

Substrate is of either p type or n type like source & drain know, I think we are not taken into account the substrate.
