Concrete cover shall not be less than the diameter of the reinforcing bar or 15mm, whichever is more. Hence the answer is E.

You are right but normally,
15 for slab.
20 for beam.
40 for column.
50 for footing.

But for mild condition and r/f dia is 12mm or less than that time it's consider 15 mm.

Generally,
Slab - diameter of the bar or 15mm whichever is more.
Beam - 25mm.
Column - 40mm.
Footing - 75mm.

According to IS 456:2000.

For main reinforcement up to 12mm diameter for mild exposure, the nominal cover may be reduced by 5mm.

It is also mentioned in code i.e it shall not be less than dia of the bar.
16mm is correct.
