For slab, concentrate cover is 25mm or not less than the diameter of the main bar so answer is 16mm.

According to IS 456:2000.
For main reinforcement up to 12mm diameter for mild exposure, the nominal cover may be reduced by 5mm.
It is also mentioned in code i.e it shall not be less than dia of the bar.
16mm is correct.

Generally,
Slab - diameter of the bar or 15mm whichever is more.
Beam - 25mm.
Column - 40mm.
Footing - 75mm.
Slab - diameter of the bar or 15mm whichever is more.
