The CG of backfill will be just above the CG of retaining wall which tends to overturn it in an anticlockwise direction.

Weep holes are provided for drainage purpose, it is not a defect. Overturning is the correct answer.

Imbalance of the i.e weak in bearing capacity do cause overturning, backfill should be given precisely so as to be effective.

How can you tell that it is only due to overturning?

In the given figure there are weep wholes so there is a defect in a vertical stem so this may be due to unfit conditions of backfill.
