Because of improper size the wall will get slide or overturned. So answer is over turning.

Lack of backfill filter media can cause more pressure. Boulder backing is also an important criterion in case of mass concrete sections. Since this is a gravity wall. Due to the heavy earth behind and high backfill height, Overturning can be considered.

Overturning causes only when the Heel position get settled right? I think it is improper drainage.
