ft = fck + kS where k = 1.65 and S = standard deviation . And as per the question 4000 kg/cm2 is equal to M400 conc that is not usually possible it should be 400 kg/cm2 and so does in case of standard deviation it should be 50 instead of 500 so the variation should be 1.25 * 500/4000 = 15.625%.

Coefficient of Variance is the percentage ratio of standard deviation to the mean.
Hence 12.5% is the answer.
