Badly mixed concrete means not uniformly mixed so result in honey combing. Segregation is due to the separation of ingredients rather it is well mixed. Segregation may be in the time of transportation and pouring.

Honey combing occurs also due to w/c ratio less than 0.35, bleeding due to excess water, so segregation is the correct one.

Honeycombing is the term used to describe areas of the surface that are coarse and stony. It may be caused by insufficient fine material in the mix, perhaps due to incorrect aggregate grading or poor mixing.
