Follow a Book.
Under 'Contents', 'Chapters' are given. So, it should be, Title => Content => Chapters => Introduction => Index.

I think Option C is Correct.
Because in a book first comes the,
Title - Title of the book.
Contents - Contents which include the Topics and Subtopics along with their Page Numbers.
Introduction - Introduction to the book or the Author.
Chapters - Chapters or Topics given in the Contents.
Index - Main and Important topics in a chapter given along with their Page-Numbers in Alphabetical Order.
But, Appendix is in the form of a table of subsidiary matter at the end of a book or document.
So, Option C is Correct.

Here index is at the begining of each chapter. Generally in books written by foreign writer. That how the chapter will go. It's topics and sub-topics.
So, Title, introduction, content, chapter, index.
So, Title, introduction, content, chapter, index.
