I think Option C is Correct.
Because in a book first comes the,

Title - Title of the book.
Contents - Contents which include the Topics and Subtopics along with their Page Numbers.
Introduction - Introduction to the book or the Author.
Chapters - Chapters or Topics given in the Contents.
Index - Main and Important topics in a chapter given along with their Page-Numbers in Alphabetical Order.

But, Appendix is in the form of a table of subsidiary matter at the end of a book or document.
So, Option C is Correct.

The correct sequence for a book would be:

Introduction
Title
Index
Contents
Chapters.

The introduction usually comes before the main text of the book and sets the stage for what's to come.
The title comes next, followed by the index which lists all the topics covered in the book.
The table of contents usually follows the index, giving an overview of the chapters and their page numbers.
Finally, the chapters are presented in order.

The table of contents gives you a list of chapters, main subject headings and is always at the beginning of a book.

An Index is at the end of a book and is a very detailed list of words or theories that can be found on specific pages throughout the book.

An appendix is a section at the end of a book which includes supplementary information which may be of interest or use to the reader.

So the answer is correct. 3,2,5,4,1.

The correct order is; 3(Title) 2(Contents) 1(index ) 5(Introduction) 4(Chapters)

You get the title on the cover of the book then after opening it, there is content and then an index, afterward introductions and chapters at last.

Then how option (c) is correct? Please explain.

Ya index is always at the last of the book. We might confuse with it because in schools we always learn that we have to make the index at first.

But this is not the correct format. Index contains all the topics with the page no in the alphabetical order at the end of the book.

After the title, the book going to have an index which going to define the content of the book.

Then Content of the book tell us about the chapters included within a book,

First of all, comes the title and we have give introduction and contents (we have give subtitle and explanation) and index (an alphabetical table of contents of a page or pointer) and finally covered the chapter.

Title ( front page of book eg Wild animals).
Contents ( about the author, acknowledgement).
Introduction ( focus parts of wild animals, what we will get to read).
