INDEX comes before chapters then comes APENDIX (a to z words of book) so the correct answer is 3. Title 2. Contents 5. Introduction 1. Index 4. Chapters.

In any book first we have a title page next to that we have contents in it next introduction but the book next what are the chapter covered in it and finally index to know where it is.

First of all, comes the title and we have give introduction and contents (we have give subtitle and explanation) and index (an alphabetical table of contents of a page or pointer) and finally covered the chapter.

Here content itself consist of index and chapters so there is no need to specify both index and chapter.
