"He is the son of the only son of my mother." Please observe the sentence that mentions "only son of my mother" it means mother has only one son that is suresh, hence the son (in photograph) of suresh means-Suresh is father of that boy.
If "only" would not have been there, it will mean mother would have more than one child (suresh) then answer would be different.

Here the said mother is the mother of Suresh. And Suresh is the only son of his mother. The boy pointed in the photo is the only son of Suresh!
