Why can't it be "I fail to understand"? Perhaps he is still in the state of not understanding, hence present perfect tense. Someone, please explain to me clearly.

In the later part of the sentence, past tense is used to refer something that has happened. So If I change 'fail' to 'failed' then, B must be changed to 'had replied'. Please confirm.

If it's 'I fail to..' it's correct.
and if we use 'I failed to..' then 'why he had replied' should be used.

He can use present tense in reference to past tense.
