Fatter than before - this sense makes the use of fatter right here. Also take exercise is an accepted use like in you take the examination. So, I feel the option D 'no error' is likely here.

Both Take and Do exercise is correct.

However, the answer 'getting fatter is correct when describing the present condition compared to the past.

We use Fatter in comparative sentence and this sentence is not comparative. That's why here using Fat.

Exercise can take a place of verb or do exercise but never heard we take exercise.
