So when the 1st verb is present (past) tense next 2 verb must be present (past).

Why shouldn't we use had in the first sentence because that action happened first also it comes with the word "when"?

This is a simple past tense so that not require use of have/had.

In case of 2 incident the first which was happened earlier should be in present/past prefect mode.

Usage of had stopped aching would be wrong because we have already used stopped to indicate simple past tense.
