The sentence should be 'He served the country with his heart and soul but got nothing in return'.

Remove 'with' because heart and soul are never preceded by a preposition.

It should be "He had served the country" but got nothing in return.

The answer should be (E). I am explaining why it should be?
WITH generally follows NOUN but doesn't follow the adverb. The correct sentence is either:
"He served the country with heart and soul but got nothing in return."
OR
He served the country hearty and with soul but got nothing in return.
