You are all wrong. Here are the two possible correct forms of the sentences: The warden stopped the student from leaving the hostel or The warden forbade the student to leave the hostel.

Forbid or forbade will follow "FROM" so the answer is option D.

Forbade is past tense and forbidden is past participle form or forbid. So option d is right since the sentence is in past tense form.
