"at" refers to an exact situation, place. etc, whereas "in" doesn't, so at is correct.

No, as the group is a singular collective noun, so were should be replaced with was.

I think the correct answer should be B because group appointed is a past participles which is a passive form.

And in passive form it should have been written like group being appointed.

Also were should be replaced by was.
