E cant be the option because if we are using did then first form of the verb is required and made is the second form of make..!!

Made = did + make. So there is no need of did and made at same time.

Did make means past simple.
How did he make an escape is actually correct not how he did make an escape. So option 'A' is wrong and C fits better?

It's embedded question structure.
Rule:
Subject + Verb (phrase) + Question word + Subject + Verb.
There "must be no" #auxiliary between "question word" and "the subject".
