4th option is not correct because when you are using quotes, then we write the actual words of speaker.

Because, whenever "said to" comes we have to change it to as "told".

So it becomes,
I told him that he was not working hard.

We should not choose option B because "told" should not be followed by "to" and "said to " itself gives the meaning of "told. ".

"Told" will become past sentence, and as we have convert into direct speech we have to use "said".
