The right answer is segregation. Honeycombing may be either due to bad compaction or segregation. But segregation is due to bad mixing.

Segregation causes due to non-uniform mixing.
Bleeding is due to excess water.
If it is not vibrated properly.
So badly mixed is nothing but nonuniform mixing, Therefore Segregation is directly attributed to a bad mix.

The needle vibrator is not perfectly compacted then concrete occurs in honeycombing. So basically bleeding and segregation is occurring in not properly mixing of concrete.

Honeycombing is the term used to describe areas of the surface that are coarse and stony. It may be caused by insufficient fine material in the mix, perhaps due to incorrect aggregate grading or poor mixing.

Honeycombing occurs due to poor compaction, bleeding occurs due to high water ratio and segregation occurs due to bad mixing of concrete. So the answer is segregation is right.
