I think all the given options are right.

Can anyone agree with me?

I think option D is right because honeycombing Is occurs due to less compaction, bleeding occurs due to excess of water and segregation is occurs due to all mixture should not mix properly but this question is wrong because of badly mixed means many types.

Honey combs due to bad compacting not mixing.

Honeycombing is due to bad compaction. Not from the bad mixing of concrete. I think segregation is the right answer.

Segregated mix gives concrete that is patchy and badly mixed.

I guess segregation, hony combing and bleeding will also be a result of badly mixed cement concrete.
