Honeycomb is due to insufficient compaction and segregation is due to badly mixed concrete and bleeding is due to excess in water.

Badly mixed concrete means not uniformly mixed so result in honey combing. Segregation is due to the separation of ingredients rather it is well mixed. Segregation may be in the time of transportation and pouring.
