(was+V3+by) is the sentence structure for passive voice. There was broadcast by all the major stations should be right. In normal tenses, (was+v+Ing) is the sentence structure. So above one is in the passive voice.

The past of broadcast is either broadcast or broadcasted. Both are in use, but broadcast is much more common, especially in the simple past but also as past participle.

Actually past simple of broadcast is broadcast in British English. But Americans uses broadcasted as second form of broadcast. We follow British English rules that's why the B is right answer.

I think due to 3rd Person Singular:

Base Form: Broadcast.

Past Simple: Broadcast/Broadcasted.

Past Participle: Broadcast/Broadcasted.

3rd Person Singular: Broadcasts.
