It will be "after leaving his office". Because if we write leaving his office it shows the current time, but sentence is showing that event has already occurred in past.

I think option A consist error because after using 'ing' form we have to use while before 'ing'.

The first past incident is said in perfect tense and the next consecutive incident is said in simple past tense. So, option A will be the answer.
