Yes even I think it should be : "he directly went to" instead of "he went directly ".

I think option A consist error because after using 'ing' form we have to use while before 'ing'.

Doesn't there need to be a comma after "After leaving his office"? (answer A).

It will be "after leaving his office". Because if we write leaving his office it shows the current time, but sentence is showing that event has already occurred in past.
