I think "conversion from scale" is the subject here & it is singular so that's why using "are" is wrong here..

Conversion is singular. So it should be "can be formulated". What do you say frends?

Its the statement in passive voice not in active voice that's why answer is correct.

Even in passive voice "are" should not be used for singular subject.

Conversion is singular. So can be formulated should be the correct answer. If are well formulated is to be considered as a correct option, then "Conversions" should be used.

It is not singular "But in all these cases" but are is not correct I think so.
