It's the rule of "past time" of "unreal conditions".
Rule:
If + Subject + #"verb in past perfect"..... + {would/could/might} + have + verb in past participle.
Had + Subject + #"verb in past participle"..... + {would/could/might} + have + verb in past participle.
Rule:

According to me, this sentence necessarily should have 'if' because it's a sort of condition.
So, the sentence should be like-"If you had told me about the problem, I would have helped you." and this is the correct usage with 'would have'.
So, the sentence should be like-"If you had told me about the problem, I would have helped you." and this is the correct usage with 'would have'.

In conditional sentences 'if you had' can be replaced with 'had you'. It makes no difference so 'C' is right.

'Had you been told' is passive. The context requires active vice.
