Given sentence in past continuous & active voice.
A) it is past perfect & not a shocked at all (all shocked is right).
B) & c) past perfect (and c in passive).
D) past perfect continuous. (and on).

Because last part of sentence is in simple past tense. So the first part be in simple past so two options are in simple past option a is not correct sentence so no correction required.

It is in simple past tense. So no need to go for optios b c & d (since they are in past participle). Also the 1st ststement is not proper. Hence no need for correction.

Sentence should be "They were all shocked at their failure in the competition" "their" should come in place of his because subject is they.

They were all shocked is correct here. As per the rules of 'all', all is placed between helping verb and main verb.

It is not possible to place had after they so (B, C, D) are out of answer. A is complete itself. So E is correct.

Is " on failure is not correct"? we generally say that on his success rit. So can we say on his failure?

Since shock is intransitive Verb, hence we must use it in Passive.
Therefore E is the correct answer.
