As per general rule in Tenses, When we describe two sequential actions occured in the past we should follow the following rule.
The action that took place first should be told in past perfect tense (Sub+had+verb3+obj) and the action that took immediately after first one should be told in simple past tense (Sub+verb2+obj).
Example:
The train had already left when we reached the station.
Explanation:
First action is train leaving its station.
Second action is reaching the station.

as in the question it was given that "she SAID" so it is in past tense so that the answer is had just swallowed
