Even if we take this sentence literally, nobody would like it if someone opens the mouth of a gift horse and inspects it. Taking another example, you gift a lovely watch to a friend to strengthen the bond between each other but he turns the watch upside down and frowns and says the watch is not branded.

Don't question the value of a gift. The proverb refers to the practice of evaluating the age of a horse by looking at its teeth. This practice is also the source of the expression "long in the tooth", meaning old.
