If the subject(your shoes) is regarding to the object(you) so no need use "by you" at the end of the sentence.

Here need is not the verb. Clean is verb. So its 3rd from applied. And to cleaned is incorrect so "to be cleaned".

Eg:.

Active- He picks me up at 10.
Passive- I am picked up at 10.

Active- I am speaking with him.
Passive- He is being spoken with.

In such cases, it is not compulsory to put the subject in passive form.
