As mentioned, the instructions are to be not one. More instructions are to be given so here were to be written.

Whenever -ing is attached to any verb then while doing actively to passive we have to use being+v3.

In the passive form we always use v3 form and for continuous being is added, hence its 'were being read '.

Whenever there is a use of continuous tense in the sentence we always use "being".

No @Sunita, "were being read out" is the only correct way to tell that sentence.

I think that answer should be "The instructions were been read out by the invigilator". But there is no option.
